EU Blue Card Germany - Application Form Completion Help

You need to fill this form in Germany in order to apply for a Blue Card

This article based on the real case of a Blue Card EU issued on an Indian programmer in Berlin.

First download the form from the website of the Service-Portal Berlin.

1. Information on the applicant

Name at birth:
if you haven't change your name, leave this field blank.

Date of birth:
use exact same format as in your passport. For example - 14/01/1980 (day/month/year).

Place of birth:
enter the geographic name as entered in your passport. If the city of birth is not entered in your passport, enter the place of birth as entered in one of the translations of your documents with Latin letters (e.g. your birth certificate, marriage certificate, etc.).

Current nationality:
I find the term nationality rather misleading. What is really meant is citizenship.
Remember it for all other fields of this form (e.g. the citizenship of spouse, children, parents).

Authorization to return to (if intered in passport):
If there are no special entries in your passport, just leave this field blank.

Do you intend to maintain your permanen residence outside the Federal Republic of Geermany? If so, where?:
This field is quite unimportant. Both options, yes or no, don't make much difference. Although, if you intend to send money abroad in the future (e.g. to children, parents, grandparents it is easier to tax these money transfers if you answer YES to this question. In this case, give the address of the future recipient.

Spouse:
If you are not married leave this section empty. In our example the martial status was already stated as single (s. page 1).

Important! The information on the applicant's spouse / registered partner also required if the persons concerned are remaining abroad.

Children:
If you have no childen, write here "no children" (or German "keine Kinder" or just "keine" if you wish).

A clear statement like "no children / keine Kinder" would probably please the German clerk, because Germans don't like to guess - does the visa applicant really have no children or did he just left this section empty by chance.

Father and mother:
Nationality is to be understood as citizenship (s. page 1). Residence: here is name of the city or residental area is sufficient.

Have you ever been in Germany before?
Carefully and in a very exact way enter the information you provided when filling in the application for a national visa.

Date of entry:
Date of crossing the border of Germany.

from:
If you came with a plane you do not need to mention the transit countires. Enter the departure country only.

Registered:
Enter the date of your registration (Anmeldung).

Current address:
Enter your postal address according to your registration (Anmeldung).

Contact:
Don't give your phone number. The phone calls is the worst communication channel with authorities.

As lomg as you have no profound knowledge of buereucratic German, the communication via phone has no advantages for you. It benefits only them because it speeds up their workflow and save them time.

Purpose of stay in the Federal...:
If you are filling ths form in English, you can replace Arbeitsaufnahme with the English word Employment.

Employer:
The name of the company is enough.

Names of relatives, educatonal establishment, references...:
If you don't know what to enter anything here, leave these fields blank.

Intendent employment:
Copypaste your position from the employment contract.

What are your means of substinence:
Write here Gehalt or Salary.

Do you have health insurance that covers the Federal Republic of Germany?
Set YES because in Germany there is a health insurance obligation (!).

What you need is the so-called statutory health insurance. This is the best case because it will be your long-term health insurance as soon as your employment begins. If you have no statutory health insurance right now, then so-called Travel insurance (Reiseversicherung) will also do.

I hereby apply for a visa or residence permit for:
Write here 4 Jahre / years if you have permanen employment contract. In case you have a fixed-term contract, add three months and then you have the date to enter here.

Example: Your fixed-term contract ends on 31.DEC.2024. In this case you apply for the Blue Card untill 31.MAR.2025.

Place and date:
Write here the city where the immigration authority (ABH*) is located where you apply for the Blue Card. Set the today's date.
